Burke Parkway Dental savings plan allows patients to get quality dental care at an affordable price. The plan covers:
2 Exams /Year (Comprehensive Exam/Periodic Exam)
2 Dental Cleanings/ Year ( Adult Prophylaxis/Child Prophylaxis ) Any doctor ordered X rays ( in office only )
20 % discount on any other dental procedures excluding Invisalign and Dental Implants.
Dental Implants:
Dental implant, abutment and crown – $3,499
( Price includes doctor ordered CBCT scan if needed )
Program Guidelines
– Patient’s portion of bill is due at the time of service
– No refunds will be issued if the participant decides not to utilize the dental plan
– NON-REFUNDABLE
Program Exclusions and Limitations
This program is a discount plan, not a dental insurance plan
It cannot be used:
– For referral to specialists
– For hospitalization or hospital charges of any kind
This plan is only honored at Burke Parkway Dental. It cannot be used at any other dental office
For Periodontal Patients, additional fee of $125 has to be paid for 2 of the 4 cleanings a year.
